We have had a change in the application process for anyone applying for Regular HEAP and or reverifying PIPP with AEP or Columbia Gas. Along with the normal requested documentations we also need a signature page signed before your application can be processed. You can pick this up at our office located at 8317 State Route 7 North, Cheshire, Ohio sometime before your appointment. If you are applying for Winter Crisis you do not need this signature page. If you are applying for Regular HEAP or PIPP and Winter Crisis all at the same time you do not need this signature page. If you cannot get to the office to pick one up we can fax or email you one.
